Look around for a free form builder; there are plenty of online form builders, such as pForm at http://www.phpform.org/formbuilder/index.php and downloadable applications (free- and shareware), such as Form Builder Lite at http://www.coffeecup.com/osx/web-form-builder-lite/

Do you know if there's a plug-in to help with forms? I want to develop something with a table-like appearance to gather client information.

I've just added a new "Sandvox Plug-ins" sub-forum under Sandvox. Please bring your Hellos and Announcements over there, too. I'm hoping to build this area out with information and issues about actually building a Sandvox plug-in.
